  Responsibilities:
  
  • 
    
  • Direct the overall operation of clinical services within the Home Care Home Based (HCHB) system
    
  • Provide oversight coordinating referrals to the agency with internal and external referral sources
    
  • Maintain continuity of patient care in a home care setting
    
  • Proactive, collaborative team member in the home care environment
    
  • Must have Oasis and Home Health experience
    
    
    Requirements:
    
    • 
      
    • Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
      
    • Two (2) years recent practical experience as an RN
      
    • One (1) year experience in Home Health required
